Patient-specific Instrumentation for Acetabular Cup Orientation: Accuracy Analysis in a Pre-clinical Study
نویسندگان
چکیده
Purpose We investigated two major factors which affect the overall accuracy of patient specific instrumentation (PSI) for cup insertion in total hip arthroplasty; 1) the fitting of PSI on the bony surface, and 2) deviation of cup impaction under a linear visual reference. Methods Using 12 cadaveric hips, we inserted Kirschner wire (K-wire) through PSI on the acetabulum for the linear reference to transfer planned cup alignment. Deviation of the K-wire was then measured. With bone models, deviation when aligning an impactor to the K-wire was also measured. Results The mean deviation of the fitting was 1.9° for inclination and 2.0° for anteversion. The mean deviation when aligning the two lines was 1.2° for inclination and 1.4° for anteversion. Discussion This PSI can transfer the pre-operative plan to the operating room with acceptable accuracy.

Comparison of Clinical, Functional, and Radiological Outcomes of Total Knee Arthroplasty Using Conventional and Patient-Specific Instrumentation
Background: Recently, patient-specific instrumentation (PSI) systems have been developed in order to increase theaccuracy of component positioning during total knee arthroplasty (TKA); however, the findings of previous studies arecontroversial in this regard. In the current randomized clinical study, the outcomes of computer tomography (CT)-basedPS (patient specific)-guided TK...
متن کاملAccuracy of Image-based Computer Assisted Surgery in Acetabular Cup Placement for Supine Two-incision Total Hip Arthroplasty
Introduction: Supine patient positioning in conjunction with twoincision minimally invasive surgery (MIS-2 incision) total hip arthroplasty (THA) increases difficulty of accurate and repeatable cup orientation in accordance with pre-operative planning. Investigators have suggested advantages of utilizing computer assisted orthopaedic surgery (CAOS) to successfully eliminate extreme outlying ace...
متن کاملResearch synthesis of recommended acetabular cup orientations for total hip arthroplasty.
Total hip arthroplasty (THA) is regarded as one of the most successful surgical procedures of modern times yet continues to be associated with a small but significant complication rate. Many early failures may be associated with poor component positioning with, in particular, acetabular component orientation dependent on the subjective judgement of the surgeon. In this paper, we compare the man...
متن کاملUsing acetabular fossa as a guide for anticipated inclination of uncemented cup in total hip replacement.
Positions of acetabular implant generally are considered to be major causative factors of dislocation. Accurate and consistent achievement of the preoperatively anticipated orientation of the acetabular cup is a great challenge in total hip replacement (THR). In the present study, we investigated the surgical application of acetabular fossa as a guide for anticipated inclination of uncemented c...
متن کاملUse of image-free navigation in determination of acetabular cup orientation: analysis of factors affecting precision.
We have been using an image-free total hip arthroplasty (THA) navigation system (OrthoPilot; Aesculap, Tuttlingen, Germany) to ensure accurate and reproducible acetabular cup orientation. In this study, the accuracy of this system in the assessment of acetabular cup orientation was evaluated by comparing the intraoperative inclination and anteversion angles presented by the navigation system an...
